Terms and conditions – I understand and agree to the below: 

ALL medical conditions must be disclosed to Bromley FC Community Sports Trust to participate in our activities.

- Where a medical condition has been disclosed, we cannot administer medication until an individual health care plan has been completed by the parent/carer.

- Where a medical condition has been disclosed, a participant cannot take part in activities unless they bring their relevant medication.

- The individual health care plan template can be found at bromleyfc.org or available upon request at community@bromleyfc.co.uk
